General Internal Medicine
Five teams exist on the general internal medicine rotation. Each team consists of an attending, a senior resident, residents and one or two medical students. Each team will be on call every fifth night and will follow patients admitted that night to the internal medicine services. Patients will be placed throughout the Hospital without geographical limitation, although the majority will be on the general internal medicine unit which is located in the west wing of the fourth floor of the DePaul Center.

Five faculty members have been employed by the department of internal medicine to provide clinical instruction and consultation on the floor. The paid faculty is devoted exclusively to teaching, a resource which is often found only in large university-based programs.

Coronary Care Unit
The CCU is a well equipped 12-bed unit located in the Seton Center at the Hospital. The CCU is ordinarily staffed by two senior residents, two junior residents and a cardiology resident. Clinical instruction and supervision are provided by a full-time, salaried cardiologist as well as by the staff cardiologists.

Intensive Care Unit
The ICU is a modern twelve bed unit located in the Seton Center at the Hospital. The ICU is staffed by four full-time intensivists and residents from various departments. It is a free-standing service with a separate call and conference schedule and separate teaching rounds. Residents are excused from all departmental responsibilities except general internal medicine clinic during their ICU rotation.

Emergency Medicine
Providence Hospital operates an active emergency room with nearly 60,000 annual visits. Providence Hospital also serves as the regional trauma center for southern Oakland County. The emergency medicine service is staffed by full-time, salaried emergency physicians and by residents from various departments. While on rotation to this service the internal medicine resident serves as the medical consultant to the emergency medicine service.

General Medicine
General medicine clinics are scheduled Tuesday through Friday from 1:00 to 4:30 p.m. Each resident is assigned to one general medicine clinic per week for the entire three years of the residency program. Patients remain the responsibility of the resident to whom they are assigned in order to provide for continuity of care. Each general medicine clinic is supervised by a paid member of the teaching staff.

Medical Subspecialty
There are three medical subspecialty clinics. Cardiology, gastroenterology and rheumatology clinics meet on either Tuesday or Thursday afternoons. Residents rotate through each of these subspecialty clinics during their second year of training. Each subspecialty clinic rotation is three months. Each subspecialty clinic is staffed by a specialist in that field who sees each patient with the residents. The cardiology and gastroenterology clinics are also staffed by the cardiology and gastroenterology residents.

Patient Care Load
First year residents rotating on general medicine service are expected to follow nine to twelve patients. Residents of any year rotating on a subspecialty service are expected to follow seven to ten patients or consults. When residents are on call, they are limited to five admits.

On Call Duties
We recognize that adequate personal time is essential to both a quality education and to mental health. Given that the demands on time are rigorous in any internal medicine residency program, every effort is made to maximize time for reading and for time away from the Hospital. As a matter of policy all residency programs at Providence Hospital seek to limit call to no more than every fourth night.

Second and third-year medicine residents share the "senior" call schedule. First-year medicine residents and residents rotating from other departments share the "junior" call schedule. Unlike many internal medicine residency programs, no distinction is made between second and third year residents for the purposes of call. We believe that most people prefer two reasonably pleasant years to the ordinary sequence of one hard and one soft year.

If all work is completed and the senior resident on the service is in agreement, residents may sign out and leave after midday conference on day on call.
