St. John Health works with HIV and AIDS patients to help manage their health as they live with the disease. The program is funded through Health and Human Services by its Grantee, the City of Detroit Department of Health and Wellness Promotion. We provide services to uninsured patients who need early intervention, diagnosis and treatment. Our mission is to increase life expectancy, decrease transmission of the disease and reduce HIV morbidity. Care includes:

Early Intervention

Patients with newly diagnosed HIV/AIDS are evaluated to determine if they are candidates for medication therapy. After evaluation, patients who are candidates for medication are then educated about the long-term commitment necessary for treatment.

Diagnosis

Patients can receive HIV/AIDS care after a diagnosis of HIV/AIDS is made. If a patient or physician has a question regarding a patient’s test result, we are available for consultation.

Treatment

Medication therapy decisions are made on an individual basis. Factors that are considered include the patient’s T-cell count and HIV viral load. After the treatment begins, patients initially make physician visits monthly to monitor the response and any side effects. Adherence to medications is important.

Case Management

Twenty-four hour case management services are provided to help patients who need assistance with housing, completing social services forms and applying for other programs specific to their needs. These services are provided in partnership with the Visiting Nurse Association (VNA).
